Philippe discusses the relationship between men’s pelvic floor dysfunction and sports rehabilitation.
Philippe is a Doctor of Physical Therapy and former Georgetown strength and conditioning coach. Philippe makes the case for why sports PTs should care about the male pelvic floor. He walks through his full evaluation process from the base of the neck to the feet, explains the obturator internus connection between the hip and pelvic floor, and shares how he has resolved persistent groin and hip cases by looking at pelvic floor involvement. He covers strengthening progressions from Kegels to sumo squats to single leg work, mobility approaches including low load long duration stretching and weighted RDLs, and why defaulting to Kegels can actually make things worse.

In this episode, we are joined by Philippe Schafer, a doctor of physical therapy practicing in New York City who specializes in men’s pelvic health. Philippe holds advanced certifications from the Herman & Wallace Pelvic Rehabilitation Institute and brings a whole body, root cause approach to treating erectile dysfunction, pelvic pain, and urinary dysfunction in men.
As someone who also has a background in strength and conditioning coaching, Philippe bridges the worlds of athletic performance and pelvic rehabilitation in a way most men have never encountered before.
Erectile dysfunction is often treated as a purely medical or psychological issue, but what if your pelvic floor muscles are a major piece of the puzzle? This conversation explores what the pelvic floor actually is and why it plays a central role in male sexual function. Philippe walks us through his comprehensive head to toe physical assessment, covering the thoracic spine, lumbar spine, hips, and pelvic floor, and explains how tightness or weakness in any of these areas can directly impact erections.
The discussion also dives deep into the mind body connection, examining how stress, anxiety, and psychological tension commonly manifest as physical dysfunction in the pelvic floor, and how addressing the physical side can actually help break the anxiety cycle.
Philippe shares what real treatment looks like, the importance of muscle balance (not just strength), how progress is measured beyond just erection quality, and who would benefit most from seeing a pelvic floor physical therapist. His closing message is one every man needs to hear: erectile dysfunction is a very solvable issue, and it is never a reflection of who you are as a man.
EIQ Men’s Podcast
In this episode, we are joined once again by Philippe Schafer, a doctor of physical therapy practicing in New York City who specializes in men’s pelvic health. After his powerful first appearance on the show, where we explored the surprising role the pelvic floor plays in erectile dysfunction, Philippe returns to take us even deeper into territory that affects far more men than anyone talks about. With advanced certifications from the Herman & Wallace Pelvic Rehabilitation Institute and a background in strength and conditioning coaching,
Philippe continues to bring the same rigorous, whole body, root cause approach to one of the most silently suffered sexual challenges men face: premature ejaculation. Premature ejaculation is one of the most shame ridden and chronically underreported conditions in men’s sexual health, and yet, as Philippe explains, it is also one of the most fixable. This conversation breaks down what PE actually is, why so many men suffer in silence, and how pelvic floor dysfunction sits at the center of a problem most clinicians treat as purely psychological or neurological.
Philippe walks us through his comprehensive head to toe physical assessment, covering the thoracic spine, lumbar spine, hips, core, and the three layers of pelvic floor muscles, and explains why tightness, weakness, poor breathing mechanics, and overall conditioning all play a direct role in ejaculatory control. The discussion also dives deep into the mind body connection, exploring how the sympathetic and parasympathetic nervous systems govern arousal and release, why shallow breathing and a tight thoracic spine can keep men stuck in a hyper aroused state, and how the anxiety cycle feeds the physical dysfunction (and vice versa).
Philippe shares what a realistic treatment arc actually looks like, including why PE treatment is uniquely collaborative and partner dependent, and emphasizes that progress rarely requires a complete life overhaul; small, targeted changes can produce major downstream effects. His closing message is one every man needs to hear: premature ejaculation is a 100% solvable issue, and the first step is simply reaching out for help.

Great overall experience and progress – more of a full body understanding of how everything ties in together than you would expect. There are others that are good, but Philippe is exceptional. Sessions are 1:1 in a private room with him, not a big factory style setting. Highly recommend.
