What is pain?
Pain is one of the most universal human experiences, yet it is also one of the most misunderstood. Most people think of pain as a simple alarm system: something hurts because the body is damaged. While tissue injury can absolutely cause pain, modern research has shown that pain is far more complex than a direct measure of injury alone. In reality, pain is an experience created by the nervous system after evaluating many different factors including tissue health, stress levels, previous experiences, movement patterns, sleep, emotions, inflammation, environment, and perceived threat.
This is important because many people continue to experience pain long after tissues have healed, while others may have significant structural findings on imaging with very little discomfort at all. Understanding pain does not mean symptoms are “imaginary” or “all in your head.” Pain is always real. However, the experience of pain is influenced by much more than tissue damage alone.
Pain Is Protective and Important
At its core, pain is designed to protect us. If you touch a hot stove, your nervous system creates pain to encourage you to pull your hand away quickly. In this situation, pain is highly useful. It helps prevent further injury. However, the nervous system is not perfect. Sometimes it becomes overly protective. After an injury, surgery, period of stress, or repeated irritation, the nervous system may become more sensitive over time. This process is often referred to as sensitization. When this occurs, the body may begin interpreting movements, positions, or activities as threatening even when significant tissue damage is no longer occurring. This is one reason why some people continue to experience persistent low back pain, pelvic pain, jaw pain, headaches, or muscle tension despite normal imaging or extensive medical testing. The alarm system remains active even after the original threat has diminished.
Pain Is Influenced by the Brain and Nervous System
Pain is not produced solely by muscles, joints, discs, or tendons. The brain and nervous system play a major role in determining whether something is perceived as threatening enough to produce pain. This does not mean pain is psychological. It means the nervous system is constantly integrating information from the entire body and environment before producing the experience of pain. In many chronic conditions, the nervous system essentially becomes better at producing pain. For example:
- Stress can increase muscle tension and nervous system sensitivity
- Poor sleep can reduce pain tolerance
- Fear of movement can amplify symptoms
- Anxiety can heighten vigilance toward bodily sensations
- Previous painful experiences can alter how the nervous system responds in the future
Imaging Does Not Always Explain Pain
One of the most important things patients can understand is that imaging findings do not always correlate well with symptoms. Research consistently shows that many asymptomatic people have:
- disc bulges
- arthritis
- labral tears
- degenerative changes
- rotator cuff tears
- meniscus tears
At the same time, some individuals experience severe pain despite relatively minimal imaging findings. This does not mean imaging is useless. Imaging can be extremely important in identifying fractures, severe structural pathology, infection, surgical issues, or other significant medical concerns. However, imaging is only one piece of the puzzle. Pain is often more closely related to how the entire system is functioning rather than one isolated structural finding alone.
Movement Matters
One of the unfortunate consequences of pain is that it often causes people to stop moving. While temporary rest can sometimes be appropriate during acute injury, prolonged avoidance of movement may contribute to:
- weakness
- stiffness
- reduced tissue tolerance
- increased fear
- nervous system sensitization
- decreased confidence
Over time, the body may become less resilient and more reactive. This is why progressive movement and exercise are often essential components of recovery. Appropriate loading helps tissues adapt, improves circulation, restores confidence, and teaches the nervous system that movement is safe again. Importantly, this does not mean pushing aggressively through severe pain. Rehabilitation should be individualized, graded, and carefully progressed based on the person’s presentation and goals.
The Body Functions as a System
The body functions as an interconnected system, and pain rarely exists in complete isolation. Limitations or dysfunction in one region of the body can often influence symptoms elsewhere. For example, hip mobility may affect pelvic floor tension, neck dysfunction may contribute to jaw pain and headaches, and breathing mechanics may influence low back or pelvic symptoms.
Factors such as stress, poor sleep, and chronic muscle guarding can also significantly alter how the nervous system processes pain and tension throughout the body. At Schafer Physical Therapy, we emphasize a systems based approach because persistent pain frequently involves multiple overlapping contributors rather than one single structural issue alone. This is especially common in conditions such as chronic pelvic pain, TMJ dysfunction, headaches, low back pain, post-surgical pain, chronic muscle tension, and overuse injuries.
Recovery Is Often About Building Capacity
Recovery is often less about finding and fixing one isolated damaged structure and more about gradually improving the body’s overall capacity, resilience, and tolerance to movement. Long-term progress frequently involves a combination of restoring strength and mobility, improving breathing mechanics and sleep quality, reducing fear of movement, building exercise tolerance, managing stress more effectively, and helping patients regain confidence in their bodies.
Importantly, recovery is rarely perfectly linear, and temporary flare-ups do not necessarily mean new damage is occurring or that progress has been lost. In many persistent pain conditions, part of the rehabilitation process involves helping calm an overly protective nervous system while reinforcing that movement can once again be safe and adaptable. Pain Is Real But It Is Also Changeable
One of the most encouraging aspects of modern pain science is that pain can change.

What is the importance of manual therapy plus exercises?
One of the most common misconceptions in rehabilitation is that recovery comes from either passive treatment or exercise alone. In reality, the best outcomes often occur when skilled hands-on treatment is thoughtfully integrated with progressive strengthening exercise and movement based rehabilitation. Modern research consistently supports this combination, particularly in the treatment of orthopedic injuries, chronic pain conditions, pelvic floor dysfunction, TMJ disorders, post-surgical recovery, and many overuse injuries. At Schafer Physical Therapy, we strongly believe that manual therapy and exercise should not compete with one another. They should work together.
Hands-on treatment, often referred to as manual therapy, includes techniques such as joint mobilization, soft tissue work, myofascial release, trigger point treatment, mobility restoration, and movement guided hands-on interventions. While these approaches are sometimes misunderstood as simply “massage,” high quality manual therapy is often far more specific and clinically reasoned than people realize.
The literature is quite clear that manual therapy can help reduce pain, improve mobility, decrease muscle guarding, improve nervous system sensitivity, and create meaningful short-term changes in movement tolerance. For example, someone with severe neck stiffness and headaches may initially have difficulty even rotating their head comfortably enough to perform exercises effectively. A patient with TMJ dysfunction may have excessive guarding and pain around the jaw that limits mouth opening and normal chewing mechanics. Someone with persistent low back pain may have developed substantial protective muscle tension and fear surrounding movement. In these situations, hands-on treatment can often help reduce irritability and improve tolerance to movement so that rehabilitation can progress more effectively.
Importantly, manual therapy is often not simply about “breaking up scar tissue” or mechanically changing tissues in isolation. The nervous system plays a major role. Skilled hands-on treatment can help modulate pain, reduce threat perception, improve body awareness, and allow patients to move with greater confidence and less guarding. In many patients, especially those dealing with persistent pain or chronic tension, this can be extremely valuable. However, manual therapy alone is usually not enough.
This is another area where the research is very clear. While hands-on treatment can provide meaningful relief and improve movement, exercise and progressive loading are often what create the long-term adaptations necessary for durable recovery. Muscles become stronger through loading. Tendons become more tolerant through progressive stress. The nervous system becomes more resilient through repeated exposure to safe movement. Balance, coordination, endurance, mobility, and confidence improve through active participation. Progression of strength, balance, and mobility exercises is what ultimately makes the body bulletproof.
Consider the example of someone recovering from shoulder pain. Manual therapy may help reduce stiffness around the shoulder and thoracic spine while improving range of motion and decreasing discomfort. But if the shoulder muscles remain weak, poorly coordinated, or unable to tolerate load, symptoms are often likely to return once the individual resumes normal activity. Similarly, a runner with hip pain may temporarily feel better after soft tissue treatment and mobility work, but unless the body develops improved strength, loading tolerance, and movement efficiency, the underlying stress contributing to symptoms may persist.
The same principle applies in pelvic floor rehabilitation. A patient with chronic pelvic pain may have substantial muscle guarding, breathing dysfunction, hip stiffness, and nervous system sensitization. Hands-on treatment may help calm symptoms and improve awareness of tension patterns, but long-term improvement often also requires restoring leg and hip strength, full hip mobility, breathing mechanics, exercise tolerance, and confidence in movement. Without that progression, patients may remain trapped in a cycle of temporary relief without true functional recovery.
One of the biggest mistakes in rehabilitation is performing the same basic exercises indefinitely without meaningful advancement. Early stage exercises are often necessary to reduce pain, improve awareness, and restore initial movement tolerance, but recovery should ideally evolve over time. The body adapts to challenge. If rehabilitation never progresses beyond extremely low level exercises, tissues and movement systems may never fully regain the capacity necessary for real world demands. Implementing full body compound exercises like squats, deadlifts, shoulder press, and heavy carries are imperative for a full recovery.
For example, someone recovering from low back pain may initially benefit from breathing drills, positional exercises, and gentle mobility work. However, if the rehabilitation process stops there, the person may continue struggling once they attempt to lift groceries, carry children, return to the gym, or resume sports. Eventually, the body must rebuild tolerance to more demanding activities. This is why progression matters so much.
At Schafer Physical Therapy, we frequently emphasize progression toward meaningful movement patterns such as squatting, hinging, carrying, rotating, pushing, pulling, and eventually returning to higher level activities when appropriate. Progressive strengthening is not simply about building muscle for appearance. It is often about restoring resilience, confidence, tissue capacity, and the ability to tolerate the physical demands of life.
The literature strongly supports exercise progression for conditions involving tendinopathy, low back pain, osteoarthritis, post-operative rehabilitation, athletic injuries, and chronic musculoskeletal pain. Gradual exposure to movement and load helps teach the nervous system that the body is capable and adaptable again. Over time, this often reduces fear, improves function, and increases overall physical capacity.
Importantly, progression does not mean recklessly pushing through severe pain. Good rehabilitation should be individualized and appropriately dosed. The correct exercise at the wrong stage of recovery can be unhelpful. Likewise, remaining underloaded for too long can also limit recovery. Effective rehabilitation often requires careful clinical reasoning regarding timing, irritability, tolerance, movement quality, and progression strategy.
Ultimately, the goal of physical therapy should not simply be to reduce symptoms temporarily. It should be to help individuals move better, feel stronger, regain confidence, and improve their ability to participate fully in life. Hands-on treatment and exercise are not opposing philosophies. They are complementary tools. When integrated thoughtfully, they can create a powerful rehabilitation approach that addresses pain, movement, strength, mobility, nervous system sensitivity, and long-term physical resilience simultaneously.
The best rehabilitation programs rarely rely exclusively on passive care or exercise alone. Instead, they recognize an important truth that both clinical experience and modern research continue to support: meaningful recovery often occurs when skilled manual therapy helps prepare the body for movement, and progressive exercise helps build the long-term capacity necessary to sustain it.
Why is sleep so important for recovery, pain and overall health?
Sleep is one of the most important yet consistently underestimated components of human health. While many people focus heavily on exercise, stretching, supplements, or treatment interventions, the reality is that the body performs some of its most critical recovery and regulatory processes during sleep. Nearly every major system in the body, including the nervous system, immune system, hormonal system, musculoskeletal system, and cardiovascular system, is profoundly influenced by sleep quality and duration. In the context of pain and rehabilitation, sleep becomes especially important.
Research consistently shows that poor sleep is associated with increased pain sensitivity, slower tissue recovery, impaired physical performance, reduced stress tolerance, and greater risk of chronic pain development. In many cases, individuals struggling with persistent pain conditions such as low back pain, TMJ dysfunction, headaches, pelvic floor dysfunction, or chronic muscle tension are also dealing with significant sleep disruption. Unfortunately, this can create a difficult cycle in which pain interferes with sleep, and poor sleep further amplifies pain.
One reason this occurs is because sleep plays a major role in regulating the nervous system. Throughout the day, the brain and body are constantly processing sensory information, physical stress, emotional stress, and movement demands. Sleep helps recalibrate many of these systems. When sleep becomes chronically insufficient or fragmented, the nervous system often becomes more reactive and less resilient. This can increase irritability, muscle guarding, stress sensitivity, and the overall perception of pain.
Importantly, pain is not simply produced by injured tissues alone. The nervous system heavily influences how threatening or uncomfortable sensations feel. Poor sleep can lower pain thresholds, meaning that activities or movements that might normally feel tolerable can become significantly more painful when someone is sleep deprived. This is one reason individuals often notice that symptoms flare more intensely during periods of stress, exhaustion, or disrupted sleep.
Sleep is also essential for physical recovery and tissue adaptation. Exercise, strength training, and rehabilitation place stress on the body in order to stimulate adaptation. However, adaptation largely occurs during recovery periods, particularly during sleep. Muscles repair, hormones regulate, inflammation is modulated, and the body restores energy reserves. Without adequate sleep, recovery capacity becomes compromised.
Athletes provide a very clear example of this relationship. Research has shown that inadequate sleep is associated with reduced reaction time, decreased athletic performance, impaired coordination, reduced strength output, slower recovery, and increased injury risk. But this principle does not apply only to high-level athletes. It also applies to everyday individuals attempting to recover from injury, improve physical function, or manage chronic pain conditions.
Sleep also appears to influence emotional regulation and stress management in powerful ways. Chronic stress often contributes to increased muscle tension, shallow breathing patterns, jaw clenching, pelvic floor guarding, headaches, and heightened nervous system activation. Poor sleep can make these patterns significantly worse by impairing the brain’s ability to regulate emotional and physiologic stress responses effectively. Over time, the combination of stress, poor sleep, and nervous system sensitization can contribute to persistent symptom cycles that become increasingly difficult to break.
Another important concept is that sleep deprivation can alter behavior in ways that indirectly worsen pain and recovery. People who are exhausted often move less, exercise less consistently, tolerate stress more poorly, and feel less motivated to engage in healthy routines. Physical activity decreases, recovery behaviors deteriorate, and overall resilience often declines. This can contribute to progressive deconditioning and reduced tolerance to movement over time.

While improving sleep alone may not instantly resolve a complex orthopedic or pain condition, consistently poor sleep can absolutely act as a meaningful barrier to recovery. In many cases, improving sleep quality helps patients tolerate exercise better, recover more effectively, regulate stress more successfully, and reduce overall nervous system irritability.
Importantly, this does not mean people need “perfect” sleep to improve. Many individuals become anxious or hyperfocused on optimizing every aspect of recovery, which can sometimes create additional stress. Instead, the goal is often gradual improvement and greater consistency over time. Small improvements in sleep quality can still produce meaningful benefits for physical performance, recovery, pain sensitivity, mood, and overall health.
Ultimately, sleep is not simply passive rest. It is an active biologic process that plays a fundamental role in healing, adaptation, recovery, and nervous system regulation. For individuals dealing with pain, stress, injury, or persistent physical symptoms, sleep is often one of the most powerful and underappreciated foundations of recovery.
